More GPs sign up to Birmingham OwnHealth
Over the telephone advice on long-term conditions including diabetes and heart disease.

More GPs in east Birmingham are signing up to Birmingham OwnHealth, which offers patients with long term conditions care, support and advice over the telephone.

It is now being rolled out to the Shard End, Sheldon, Stechford and South Yardley areas of Birmingham.
 
Birmingham OwnHealth supports members with conditions such as diabetes, COPD, chronic kidney disease, stroke, high blood pressure, heart failure and coronary heart disease.

Also, those aged over 65 who would like help with a range of health conditions to make positive changes to their health and lifestyles and learn how to manage their conditions better.

Birmingham OwnHealth provides members with access to a qualified Care Manager, who helps them to take control of their own health, make positive changes to their lifestyles and manage their conditions better.
 
Caroline Nolan, Divisional Director for Strategy and Redesign at NHS Birmingham East and North, said: "Feedback shows that Birmingham OwnHealth is already improving the health of our existing members by giving them the power to manage their conditions better."

"They have made positive changes to their lifestyles, reduced their weight and improved their cholesterol, blood pressure and blood glucose levels, and they report that they don’t need to go to their GP or hospital as often."
